Introduction
In addition to traditional primary care, we offer a comprehensive, structured yet individualized program to optimize healthspan. This program is typically done via telemedicine with 6-8 visits over the course of the first year. The initial visit is 90 minutes and subsequent visits are 30-60 minutes.
Each visit is dedicated to 1-2 of the core healthspan topics. We’ll prioritize them based on your preferences and needs as discussed at the initial visit. Prior to each visit, we will work together to obtain relevant data (labs, coronary CT, DEXA, CGM, exercise testing, whole genome sequencing, etc) and then develop a plan with measurable benchmarks which we will revisit throughout the year.
We will use a custom-built tracking platform to track and analyze your healthspan data. The platform currently interprets about 250 types of data encompassing labs, vital signs, DEXA values, raw CGM data, coronary/vascular imaging, strength and aerobic fitness, diet, sleep, wearables, etc. Where appropriate, age/ethnicity/sex-adjusted population percentiles are calculated based on published studies and ideal ranges provided. Dashboards for each major healthspan domain allow you to visualize your progress. You will be able to use this system during and between visits to input data and track progress.

What is unique about this program?
Compared to other programs such as Biograph or InsideTracker, this program is unique in the combination of:
- frequent, individualized visits with a uniquely trained and experienced internal medicine physician
- a monitoring platform which combines multimodal data including labs, imaging, fitness and strength metrics, wearables, etc
- the ability to directly prescribe medication (eg, GLP1 agonists, rapamycin) or place referrals (eg, to cardiology or physical therapy) when appropriate
- a holistic approach that combines the best of Medicine 2.0, Medicine 3.0, and integrative approaches
- incorporation of cutting edge technologies and wearables such as Cleerly AI-based coronary CT angiography, 24 hour blood pressure monitoring patch, EEG based lab-quality at-home sleep study, whole genome sequencing, etc
How much does the program cost? Do you take insurance?
Please inquire about current rates. We are out of network with all insurance providers but can provide superbills after each visit that can be submitted to your PPO insurance. You can ask your insurance company what they reimburse you for an out of network 99215 internal medicine office visit. Labs, imaging, etc are not part of our fee but your insurance will cover them as they do currently.
An hourly rate option is available for patients who want to work on specific higher complexity issues requiring more frequent visits or significant research. What if I want to be seen in person?

What if I want to discuss issues that aren’t listed on the core healthspan topics page?
General primary care issues (for example, coughs, abdominal pain, knee injuries) are not covered under the healthspan program but would be covered under the concierge primary care program (see previous FAQ topic). Some borderline areas include existing cardiovascular disease, hypertension, fatty liver, weight loss, or diabetes which may be covered on a case-by-case basis as a means to prolong healthspan. If one of these borderline issues is your primary reason for signing up, please schedule a complimentary Meet & Greet visit first to discuss the best option.
What methods of communication are used?
For telemedicine visits, we typically use encrypted Zoom calls but FaceTime is available if necessary (limitation: no screen sharing). For between-visit communication, Epic MyHealthOnline secure messaging, phone calls and email are all possible. Messages are usually responded to on the same business day but sometimes may take 1-2 days. You must continue to contact your regular primary care doctor for urgent concerns.
How to schedule a visit?
You will be given a link to schedule visits at your convenience. Availability is generally within a few days.
How can I cancel my membership?
You may cancel your membership at any time via email or secure message if notice is given before the first day of the billing cycle. For months during which a visit has already occurred, refunds are not available.
How much does additional testing cost?
We will individualize the testing you get based on your needs and budget. Most of the lab testing is covered by insurance and if not covered usually costs $25/test. Coronary calcium scans can be as little as $100 out of pocket if not covered by insurance. Coronary CT angiography with Cleerly analysis is $1,500-1,800 in total out of pocket without insurance; the CTA scan itself is sometimes covered by insurance with concerning symptoms but the Cleerly analysis is not covered by any insurance currently. DEXA scans are about $60. VO2max testing is $100-300. Prenuvo MRI is $1,000-2,500. Grail Galleri cancer screening test is $950. 24 hour blood pressure monitoring patch is $150.
